We accept many insurance PPO plans. Please see the list below. If you don't see your insurance plan listed, we most likely can accept your insurance, as an out-of-network provider. Please call our office for details at (813) 891-1212. We'll gladly file your insurance claim on your behalf. Please bring your insurance card with you on your first visit to our office
Insurance companies typically only cover a portion of your total treatment cost, as a benefit to you. We ask that you pay your portion of the bill at the time of treatment We will do our best to try to estimate your portion. Any remaining balance not paid by your insurance company, is the patient's responsibility.
